My Rec Pass Survey

Thank you for your interest in the My Rec Pass project. The survey will take approximately 15 minutes to complete. Questions marked with an asterisk (*) are mandatory.   

If you have previously registered for an Engage Ottawa account, you will need to login before beginning the survey, otherwise you will receive a message indicating your email is already in use.

By clicking “Start” you consent to participate in the online questionnaire. The questionnaire is anonymous, and your participation is voluntary. Any contact information provided will be kept separate from your responses.
