Older Adult Plan Roundtable 2026 - Partner membership form

DESCRIPTION

The Older Adult Plan Roundtable will guide and provide overall leadership, coordination, and oversight of the implementation of the Older Adult Plan. Community agencies will play a key role in advancing age-friendly initiatives, sharing expertise, and collaborating with City staff and older adults. The Roundtable will include representatives from City departments, community organizations, and older adults. Please take a moment to read the Terms of Reference before filling out this form.

APPLICATION AND SELECTION PROCESS

Application deadline: Wednesday, December 3, 2025

Selection: A selection committee will review the applications and ensure equitable representation of Ottawa’s diverse older adult population.
Results: Successful candidates will be notified by email before December 17, 2025

ELIGIBILITY CRITERIA

Organizations must:

Be a community-based not-for-profit organization located in and serving residents of Ottawa
Provide services or support initiatives for older adults in Ottawa
Be interested in equity-focused collaboration and collective impact
