As indicated in your EarlyON Child and Family Centres Contribution Agreement with Children’s Services, City of Ottawa, EarlyON service providers are required to report Service Data Elements. This information helps Children’s Services in planning and ensuring access to essential services, like EarlyON Actual EarlyON Service Data Elements for January 1 to December 31, 2025, are required. 

This survey collects information from child care service providers whose reported revenue from fees for 2025 was below the expected revenue from fees based on the Ministry’s requirements. Responses will help Children’s Services assess whether extenuating circumstances affected enrollment and parent fee revenue, and determine eligibility for a one-time recalculation through the 2025 year-end reconciliation process. Please complete one survey for each eligible site. 

We are gathering feedback from the child care sector to better understand what information and support would be most helpful as Cost-Based Funding is implemented. This short 15-minute survey is an opportunity to share your questions and areas where additional information or support would be helpful. Your feedback will help us identify common themes across the sector and inform the content of an upcoming Cost-Based Funding information session planned for the end of September, ensuring it addresses the topics most relevant to child care service providers.
